Please provide contact information for an adult that is NOT one of the parent/guardian’s. This information will be used in the case of an emergency when neither parent/guardian can be contacted.

Adults Authorized to Pick up Child

You may provide other adults in addition to the parents listed above that are authorized to pick up the enrolled child. The authorized person(s) will need to show proof of I.D. before the child is released to them.

Permissions

We understand that some parents would prefer their child’s photos not to be published. Please understand that while at Camp Quest, your child may be photographed by official Camp Quest photographers and those photos will be kept safely in the private possession of Grace Point Fellowship. Please check yes or No below to indicate if Grace Point Fellowship has your permission to use your child’s photo in print, web, and other electronic publication methods.

I understand and agree that my child has permission to participate in age-appropriate Camp Quest activities. If you select “No” to this question, your child is still eligible to attend Camp Quest but will be excluded from recreational activities. If your child has a special case that requires more explanation please contact us at (956) 454-3383 and let us know of the situation. We will do our best to meet the individual needs of each child.
